Overview
The 30W SmartXide DOT™ and 50W SmartXide DOT HP™ allow the treating physician to attain high peak powers needed for safe, effective and comfortable deep wrinkle reduction treatments. Combined with the unique SmartPulse™ Technology and SmartStack™ feature options, the SmartXide DOT™ series is the clear leader in fractional CO2 technology.
The SmartXide DOT™ laser with DOT Therapy is the ideal modality for the treatment of dyschromia, skin laxity and texture, wrinkles, dermal lesions, photoaging, acne scarring and scar reduction on all skin type patients. Off face uses of the device include treatment of trauma scar revisions, striae revisions, 3rd degree burn revisions, ablation of nevi and sebaceous keratosis and more. Dermal Optical Thermolysis (DOT) is the ideal approach to skin rejuvenation. It creates thousands of microscopic perforations, which induce immediate skin tightening and stimulate new collagen growth, improving the skin's texture and tone to improve fine lines, deep wrinkles and acne scars.
DOT Therapy is the newest, state-of-the-art procedure for laser skin resurfacing. Using the SmartXide DOT™ CO2 laser system will allow the treating physician to take the technology to a new level with standard and fractional delivery methods to customize each treatment area on every patient. DOT Therapy can be used for fractional laser resurfacing on the face as well as non-facial areas like the neck, decollete and hands to effectively treat sun damage, dyschromia (pigmentation), rhytids (wrinkles), skin laxity and acne scarring.
The SmartXide DOT™ with SmartPulse™ delivers the highest ablation power to tissue allowing for the cleanest ablation available in the industry. At over 250 W, the SmartXide DOT™ has 10 times the average power to tissue of other systems on the market. The SmartXide DOT™ is the only system with SmartPulse™, a modulated superpulse technology that adds additional low power pulse with user variable coagulation. The tissue dwell time, as it is adjusted on the control screen, is a combination of ablative and controlled coagulation tissue effects that allows the user to deliver both short term ablation, 50 microseconds, with controlled coagulation of the target tissue while best maintaining safety margins.
The SmartStack™ feature for the SmartXide DOT™ gives it the ability to apply a microburst of up to 5 pulses into each fractional spot. Each micropulse has the very high SmartXide DOT™ peak power for clean ablation at 50 microseconds each. As a result the SmartXide DOT™ can ablate several times deeper than before and as deep as any clinical indication requires including scars and very deep wrinkles. Adjustable dwell time can still be controlled to allow for ultimate control of the procedure. This allows the treating physician to be more aggressive as you become comfortable with SmartXide DOT™ therapy treatments. The microburst feature will also allow the treating physician to deliver deeper penetrations on all skin type patients using shorter pulse widths while maintaining safer treatment parameters and minimizing patient discomfort.
The versatility to adjust your input power and dwell time allow the user to adjust the pulse energy over the treatment area and selectively control the depth of penetration into the target tissue as well as controlling the lateral thermal damage delivered to adjacent tissue promoting coagulation and tissue constriction. The ability to adjust your spacing allows you to control the percentage of tissue to be treated promoting shorter or longer healing times based on the expectations of your patients.
Unlike other skin resurfacing CO2 lasers, the SmartXide DOT™ laser system is unique because the treating physician is able to selectively control the spacing or "pitch" of the DOTs. This control allows you to "fractionate" the laser energy over a customized pattern of micron-sized DOTs to maximize or limit the thermal density on the treatment tissue. The untreated tissue remaining after DOT Therapy promotes faster healing times that can be customized with each patient depending on their schedule.
The Computer Pattern Generator (CPG) has 38 pattern variations to allow an even distribution of laser energy over the treatment areas. Button touch controls on the CPG allows the user to adjust the pattern shape, size and ratio with the touch of a button eliminating the need to return to the main control console for all changes. The SmartXide DOT™ CPG scans 2 to 3 times faster than that of the Lumenis or Sciton systems, allowing for decreased treatment times and maximize patient throughput.
The SmartXide DOT™ offers 3 scanner delivery methods, standard, interlaced and SmartTrack™, to allow the treating physician to better manage patient discomfort levels.
Handpieces are also available that allow the physician to free-hand sculpt or ablate tissue using continuous or pulsed delivery methods.
The SmartXide DOT™ CPG offers a hybrid spot size compared with other fractional resurfacing laser on the market. The 350 mm spot allows the treating physician to penetrate deeply into the target tissue to promote new collagen growth as well as improve epidermal dyschromia using only one CPG eliminating the need to change CPGs in the middle of your treatment. The hybrid spot size in combination with SmartPulse™ dwell timings does not cause the bleeding nor the discomfort commonly associated with other fractional CO2 systems available on the market promoting a better patient response.

Most patients should tolerate the procedure well with the use of a topical anesthetic and local injections, although anti-anxiety medications may also be suggested provided the patient is able to have a driver present post treatment.
An independent study was performed that evaluated clinical impressions of 9 fractional systems available on the market today and their results were published in the May 2009 issue of the JDD. The SmartXide DOT™ received high marks in all areas of treatment and received the highest overall improvement rating when compared to other fractional modalities.
